Health Improvement Partnerships

A Health Improvement Partnership is a local collaborative organized to improve the health of the respective community. Each Health Improvement Partnership is unique in size, membership, operating structure, and area of focus.

Health Improvement Partnership contact information is available by county (PDF) and by health district/region (PDF).

To become a new partnership or to annually update current partnership information, a representative from the Health Improvement Partnership needs to complete the Partnership Registration and Annual Registration Report (PDF) and send it via email to ra-dhhipp@pa.gov.

Health Improvement Partnership Newsletter

Published bimonthly, the Health Improvement Partnership newsletter contains news from DOH, national and state partners, health improvement best practices, funding opportunities, Health Improvement partner highlights, etc.
