School Nurses
At no cost, the Pennsylvania Department of Health is offering school medical staff view-only access to the PIERS in order to view the immunization histories of enrolled students within their respective school.
In order to obtain view-only access to the PIERS, each medical staff member of your school will need to review the Confidentiality Policy and then complete a PIERS User Agreement (School).
All completed PIERS User Agreement Acknowledgment forms for your school should be mailed together to the following address:

After you have completed and submitted the User Agreement Acknowledgment forms, a PIERS staff member will mail your account information to your school.
