Funeral homes may fax or scan the completed Medical Certification Worksheet to the Death Registry Unit. Allow 2 business days for processing.
For amendments, fax the appropriate “Request to Amend a Death Record” form to the following number. See SRN 2020-17 for forms and details.
Funeral homes may fax or scan Funeral Home Applications for Death Certificate qualifying for a fee waiver.
