Community-Based
Health Care Program (CBHCP)

Goals of the CBHCP include:

  • Expanding and improving health care access and service;
  • Reducing unnecessary utilization of hospital emergency room services; and
  • Encouraging collaborative relationships among community-based health care clinics, hospitals, and other health care providers.
