Pennsylvania Electronic Death
Registration System (EDRS)

Pennsylvania is implementing an Electronic Death Registration System (EDRS) to report death events in Pennsylvania. EDRS is a web-based application that simplifies the data collection process and enhances communication between funeral directors, local registrars, coroners/medical examiners and certifying health care providers as they work together to register deaths online.
 
The Bureau of Health Statistics and Registries (BHSR) is implementing EDRS with a phased approach. Phase 1 focused on bringing onboard all funeral directors, local registrars and coroners/medical examiners. Phase 1 officially concluded in the summer of 2017. Phase 2 will occur at a future date and will focus on onboarding medical certifiers.

TIP OF THE MONTH - MAY

Creating an Amendment / Amending a Case:

- You can amend a case only after it has been registered

- A properly completed amendment will have an "Amendment Pending Approval" status and you should receive a pop-up message stating "press OK to submit this ammendment."

- A "Keyed" status indicates the amendment was not completed, or completed improperly, and must be redone.

- Each amendment is reviewed for approval. Allow one full business day for the amendment to be approved.

EDRS STATEWIDE PARTICIPATION
Training has already been conducted for funeral homes, coroners/medical examiners and local registrars throughout Pennsylvania as part of Phase 1. In advance of Phase 2, many licensed health care facilities have requested and received training as well. The counties in green on the map below identify areas within Pennsylvania where EDRS has been implemented by at least one licensed health care facility.

EDRS TRAINING OPPORTUNITIES
Training is available to coroners/medical examiners and funeral homes. Webinars will be conducted for those who were unable to attend training during Phase 1 or would like a refresher. If you are interested in receiving EDRS training, you will first need to register for an EDRS account.

Licensed funeral directors who attend a live webinar for "Funeral Home EDRS Training" will receive continuing education units (CEUs) as approved by the Department of State. The CEU Acknowledgment and Attestation Form (PDF) must be completed within two weeks of training. Instructions for submittal are included on the form.
 
Training for Medical Certifiers
BHSR will be rolling out training to medical certifiers under Phase 2 of this project. We anticipate that Phase 2 may start at a future date. If your organization is interested in receiving training prior to the official launch of Phase 2, please complete the online survey for Medical Certifiers Interested in an Early Adoption of EDRS. At the present time, BHSR is scheduling a planning meeting with each organization within two weeks of receipt of the online survey.

HOW TO REGISTER FOR AN EDRS ACCOUNT
Pennsylvania's EDRS currently utilizes the DAVE™ system. To access EDRS you will need a DAVE™ account. Based on where you work, please see below for the process to register for an account.
 
FUNERAL HOMES AND CORONER/MEDICAL EXAMINER OFFICES
If you work at a funeral home or coroner/medical examiner's office, please follow the steps below to register for an account and start using EDRS.
  • Review the DAVE™ User Agreement and Confidentiality Policy.
  • Complete the DAVE™ User Account Request form (HD01591F). You may fax or email the completed and signed form to the Bureau of Health Statistics and Registries. The fax number is 717-265-8383 and the email address is RA-DHEDRSUserAcct@pa.gov.
  • Depending on your training needs, one of the following will apply:
    • If you indicated on the DAVE™ User Account Request form that you need training, you will receive a call from the Bureau of Health Statistics and Registries to schedule training. After you are trained, you will receive your log-on credentials via the unique email address you provided.
    • If you indicated on the DAVE™ User Account Request form that you have had training or will receive it from someone at your facility, you will receive log-on credentials via the unique email address you provided.

If you work at a medical facility that has not yet adopted EDRS but is interested, an authorized representative from your facility may express their interest by clicking here to complete a survey. Upon completion of the survey, a representative from the Bureau of Health Statistics and Registries will contact your facility to begin the process of adopting EDRS. Once your facility has adopted EDRS, you may register for an EDRS account through your on-site EDRS facility administrator.

FORMS

  • DAVE™ Facility Account Manager Request Form HD01592F, 2/2016 (PDF) – Organizations that are using EDRS and have 10 or more employees should complete this form in order to be set up with the facility account manager role. This role will enable your organization to have a local site administrator who is able to activate and deactivate user accounts for your organization.
  • DAVE™ User Account Request Form HD01591F, 9/2016 (PDF) – All users requesting access to EDRS are required to complete this form. The form must be provided to BHSR at time of account creation. Organizations set up with the facility account manager role are required to keep signed forms of all end-users on file at their local office.
  • CEU Acknowledgment and Attestation Form5/2017 (PDF) - Licensed funeral directors who attend "Funeral Home EDRS Training" will receive three continuing education units (CEUs) as approved by the Department of State. This CEU Acknowledgment and Attestation Form must be completed within two weeks of training. Instructions for submittal are included on the form.
  • Fax Cover Sheet for Medical Certification Worksheet9/5/2016 (Word) – This fax cover sheet is to be used by funeral directors (who have adopted EDRS) to transmit the medical certification information to BHSR (from medical certifiers who are not yet using EDRS).
  • Funeral Director Worksheet11/2015 (Word) – This worksheet is an optional tool that a funeral director may use to gather the required EDRS information on a decedent prior to entering the information into EDRS.
  • Medical Certification Worksheet, 9/5/2017 (PDF) – Medical certifiers (who are not yet using EDRS but are working with a funeral home who has adopted EDRS) are encouraged to use this form for reporting a death (instead of using the Certificate of Death H105-143 form).
  • Rejected Death Record Fax Transmittal (Word) – This form is an optional tool that a funeral director may use to return a Medical Certification Worksheet (or Certificate of Death form) to a medical certifier due to incomplete or inaccurate information.

HOW TO ADD A FACILITY TO THE EDRS LOOKUP TOOL
EDRS has a facility look-up tool for place of death. When searching for a facility as an authorized user, please perform a wildcard search using the percent sign for the wildcard. For example, if entering %har% as your search criteria, all facilities that contain “har” anywhere in the facility name will appear in your search results.
 
If you are unable to find the facility after performing a wildcard search, please submit an email to us requesting that we add the facility. Send the email to RA-DHDeathSupport@pa.gov and include the following details:
 
  • facility type (based on the facility type options available in EDRS);
  • complete facility name; and
  • facility address, including zip code.
We will inform you as soon as the facility has been added to EDRS.
